Photo, video, testimonial and quotation consent form

At Anchor, we produce a wide range of materials to tell people about our housing and care services and to raise awareness of issues affecting older people. As part of this, we take photographic images (moving and still) of residents, their relatives or friends and Anchor colleagues. We use case studies and testimonials which can include these images and other personal data (such as name, age, quotations and other details where appropriate) to enhance and illustrate our media releases and marketing materials.

By providing your details below, you are giving us your permission to use your images and any personal information, testimonial or quotations you supply to us for our publicity to reasonably promote or advertise Anchor for as long as we have a legitimate interest in doing so.

This information will be publicly visible for use in our media coverage and promotions, such as printed publications; adverts; audio-visual and electronic materials: on the internet, social media; public relations; display materials and any other media we may use in the future. To do this we may also use other organisations, such as promotional or public relations companies, or marketing organisations. The images will not be used for any other purposes.
 

Consent to use of photographs/videos/testimonials and quotations

I give my consent to Anchor, its nominated photographer, member of staff or other person acting on its behalf and in connection with its business, to take photos/videos of me, use testimonials, quotations or case studies that I have provided, for the purposes described above.

I confirm that I am not seeking and will not seek payment in cash or in kind, and that I have no copyright interest in the photos/videos of me or any child of whom I am parent or person with parental responsibility.

Storage of photographs, videos and consent forms

Photos, videos and consent forms will be held in accordance with data protection legislation. They will be stored securely, which may include storing them electronically, for a period of up to 10 years, and then deleted.
 

Withdrawing consent

You have the right to ask Anchor to remove your image. Upon request, we will securely delete images stored on our systems and electronic materials on the internet; social media and any other media we may use in the future. We will not be able to withdraw printed publications that are already in circulation.

If you wish us to stop using a photo or video of you, please call our Data Protection Team on 01274 026141 or email [email protected]. Anchor will action your request within 30 days.
