We understand there are a variety of reasons why some older people may not be able to return homes straight away following a hospital stay.
And we recognise the challenges health professionals face on a daily basis to ensure hospital beds are optimised and patients are given the support they need to regain their independence following an episode resulting in a hospital stay.
