Helping to speed up hospital discharges and preventing hospital re-admissions

An older lady flower arrangingWe understand the challenges you face on a daily basis to ensure hospital beds are optimised and that your clients are being given the care and support that they need to re-gain their independence following an episode which results in a hospital stay.

At Anchor we are ideally placed to help support the transfer from hospital to close care or a care home environment – dependant on the assessed needs of the client.

Our teams understand the importance of re-enabling our customers to achieve their independence. Our broad range of services from short stay convalescence to long term palliative care ensures we are able to offer the right care, in the right place, at the right time. We work closely with all community teams to keep hospital admissions and re-admissions at their lowest.

Through our proactive and supportive approach we promote both physical and mental health so that older people can live full, happy and worry free lives in the company of caring, supportive people. Care villages

Having listened to our professional partners, we have embraced the need for care villages within our portfolio. Our new care villages at Bishopstoke and Hampshire are designed to ensure older people have excellent choices of accommodation from buying and renting housing with care and support, to expertly designed care homes, offering high standards of care in homely environments. Properties to rent or to buy

We offer a range of properties across England which have been designed for older people who prefer and are able, to live independently with some support if needed. Many of our properties have a manager on-site to offer support and also 24-hour emergency call systems. Find out more about our retirement housing.

Care homes
